Helping people talk about and document their end of life care wishes.

Documenting how you want to live at the end of your life is an important part of your overall life planning. We help you talk about and document them with your loved ones so they have clarity when it’s needed most.

Documenting your wishes brings clarity and removes stress.

For many, creating a will or long-term financial plan just makes sense. But what about preferences around your end of life care? Even though talking about dying is tough for some, creating a document about your wishes is as important as any other planning you do, and we have seen just how powerful it is for families.

We give you tools to start the conversation, and when you’re ready, our experts guide you and your loved ones through three conversations to document your wishes. Through our facilitated conversations, we create a CareDecisions Portfolio – a 20-30 page document that covers what’s important to you before and after death as well. It is written with your own words, and includes all the healthcare-related state forms your medical teams and loved ones will need.

Give Clarity

Documenting your specific preferences lets your loved ones know exactly what you want.

Be Heard

If you can’t speak for yourself, know that you’ve enabled your loved ones to speak for you.

Feel Peace

Remove doubt and stress for loved ones during a difficult time, especially after you’re gone.

I know I need to talk about end of life but just talking about it made me uneasy. This process made a difference. Now I know what I want, and it’s recorded. I made a copy for everyone in my family.
